KĀYA Prahran Guests

By submitting the following form as a guest of KĀYA Health Clubs, you acknowledge and agree to the following conditions of entry:

a) Participation in any activity in the club is at my own risk. I do not have any existing condition which may prevent me from participating in my chosen activity. I have made an instructor aware of any injuries or predisposing condition which may affect my safety and/or health as a result of participating in activities.

b) I will be responsible for any damage to KĀYA property.

c) I will not hold KĀYA Health Clubs, its employees and/or agents responsible for any injury or loss sustained as a result of my activities while in the club, except to the extent that such occurred as a result of wilful default or negligence on the part of the Company, its agents or employees.
